Home Care Services

Home is where the heart is, and with Tri-Valley’s help frail elders wishing to stay home may do so. The right amount of essential services are provided to support safe community living.

We also work with caregivers to provide respite, which gives the caregiver an opportunity to take a breather so that they may continue to be effective.

Our State Home Care Program is designed for the very purpose of allowing elders to remain at home. Please call Tri-Valley if you are 60 or older and feel you can no longer stay home safely without some assistance or respite.

Individuals under the age of 60 with a physician’s documented diagnosis of Alzheimer’s Disease and who meet eligibility criteria including having a caregiver are also eligible to receive Respite Care Services.

We will set-up a time for a Care Manager to come out and visit with you. The Care Manager will discuss your physical abilities, health status, income eligibility and needs. Once eligibility and need is established, you (and your family) may review the available services.

A Care Plan will then be developed for you which may include:

• Light housekeeping
• Laundry
• Shopping
• Meal preparation
• Home-delivered meals
• Money management assistance
• Transportation to medical appointments
• Personal care such as bathing.
• Visitors and telephone reassurance
• Personal emergency response

Some services have no cost, most require a co-payment or a suggested donation; some call for a full fee.

Your Care Manager will set-up your services and continue to visit you to see that your needs are being met.

A variety of other services outside the home and may include: Adult Day Health, Social Day Care and Dementia Day Care.
