Before entering
long-term care, Tri-Valley helps evaluate your situation so you
can make an informed decision about yourself or your loved one.
Once a decision is made, you remain empowered, still have a say
in certain matters affecting that nursing or rest home placement.
Tri-Valley helps elders and disabled people exercise their rights
fully and independently by providing information and advocacy, including
the option to move out of that situation. Here’s how:
Pre-Admission Screening
A nurse helps determine if a person is medically eligible
for nursing home placement paid for by Medicaid/MassHealth. All
screenings involve an assessment by a nurse with medical information
provided by the client’s doctor or a certified home health
agency involved with the elder. (click

The
Ombudsman Program
A trained, certified representative works on your behalf
to provide you with personalized information that helps you retain
and exercise your rights. This advocate also works for changes that
help you and others in the long-term care system. (click

Helping People Return Home
The Comprehensive Screening and Service Model (CSSM) process
provides the potential for Medicaid recipients in nursing homes
to return to their home communities. Seniors 60 or older may be
eligible for Tri-Valley Home Care Services to help support your
