Eligibility Screening

Whether it’s
getting assistance with personal care, day health services, or taking
a proper look at the health advantages of nursing home care, Tri-Valley
Inc. is there for elders.
Our Coordination
of Care Department nurses conduct screenings that provide valuable
information, to see that the right health services are delivered
to each individual. Our staff of registered nurses coordinates with
other medical professionals and services that provide health care,
and other Tri-Valley programs that keep seniors comfortably at home.
When it becomes
advisable to at least think about nursing home placement, our staff
gathers the information to determine if a person is medically eligible
for nursing home placement paid for by Medicaid/MassHealth. All
screenings involve an assessment by a nurse with medical information
provided by the client’s doctor or a certified home health
agency involved with the elder.
The nurses may
also screen for adult day health services, or for an electronic
Personal Emergency Response System that an elder in the community
who is on Medicaid may require.
Further, the
nurse may be able to suggest other options that would not involve
nursing home placement. If someone is not nursing home-appropriate,
a Care Manager may assist in developing a community-based care plan.
