Our Adult Family Care program offers an alternate living arrangement,
a quality home life for people who require some assistance with
activities of daily living, but do not need or desire placement
in an institutional setting. An individual is placed in the home
of a caregiver, and the goal is that the person becomes and feels
like a member of a new family.
The
program is ideal for someone who wants an accommodating family close
by, but perhaps no longer has family caregivers able to see to his
or her needs for various reasons.
Participants
must:
• Be at least 16 years old.
• Have MassHealth or be able to private pay.
• Have a medical diagnosis.
• Require assistance to complete at least one of the following:
bathing, dressing, grooming, feeding, toileting, transfers or ambulation.
• Be drug/alcohol free for at least one year.
• Not be a risk to themselves or others.
• Be able to pay the room and board with money left to cover
personal expenses (medications, toiletries, clothing, etc).
Caregivers are adults with a private room available who are able
to provide care including transportation, medication administration
and assistance with daily living. They receive ongoing training
and reimbursement, as well as 14 nights of respite per year.
Once a referral is received for either a participant or caregiver,
an assessment is scheduled to determine individual interests and
abilities. The parties meet, and if this visit goes well and the
parties are in agreement, a weekend stay can be arranged. If all
goes well, a placement can occur.
The
goal of the AFC program is to create appropriate matches so that
the placement will continue indefinitely. The hope is that the participant
becomes a valued member of the caregiver’s family, rather
than a “resident.”
