Care Management: Whatever It Takes

Tri-Valley Inc. coordinates many services to help people function well while continuing to live in their homes, even people with complicated needs. Some of these services are directly provided by our agency. Others are overseen by our case management staff through formal and individual arrangements with Tri-Valley.

A care manager will be assigned to each individual age 60 or over or under 60 with a diagnosis of Alzheimer’s disease and a caregiver. The Care Manager will complete an in-home assessment of the person’s physical abilities, income and needs. Then the Care Manager, working with the Care Manager Supervisor and a nurse, will develop a plan to provide services that will help any eligible elder continue to live at home. Services may include: light house keeping, laundry, shopping, meal preparation, home delivered meals, transportation to medical appointments and personal care such as bathing
